I agree to hold harmless and waiver any rights of liability towards She Rock She Rock (SR2) and agree to assume any risk of bodily injury (including hearing damage*) or property damage.

During the period of camp, I hereby give permission for the staff of SR2 to administer appropriate medical attention to me in the event of an accident, illness or injury. I will be responsible for any and all costs of medical coverage and treatment provided not covered by insurance.

I agree that SR2 is not liable for any equipment that is damaged or broken in the course of the camp due to the actions of myself or others. 

I grant permission to SR2 to use images of me from camp. Use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of me for use in materials that include, but may not be limited to printed materials such as brochures and newsletters, videos, and digital images such as those on the She Rock She Rock website.

I give unrestricted permission for my image to be used in print, video, and digital media. I agree that these images may be used by She Rock She Rock for a variety of purposes and that these images may be used without further notifying me. I do understand that my last name will not be used in conjunction with any video or digital images.

*Prolonged exposure to noise over 85db for more than 8 hours at a time has the potential to cause hearing loss.  At camp, we will provide ear plugs for free, demonstrate their usage, and advise all campers to wear them during any amplified classes, rehearsals or performances.
